If there is ever a time when you need gynecologic surgery, it’s good to know that there are now minimally invasive surgical solutions for most gynecologic problems. Procedures that do not require incisions
Dr. Garcini is skilled in hysteroscopy techniques, where procedures are performed through natural orifices, such as the vagina and the cervical opening. Among these procedures are removal of endometrial polyps and uterine fibroids from the uterine cavity, endometrial ablation or resection and the Essure® permanent contraception technique.
These are procedures by which the internal cavity of the uterus is inspected with a thin fiberoptic lens and a light source. The lens is inserted through the cervical os (the opening of the vagina into the uterine cavity). Fluid is introduced in order to distend the cavity for better visualization. Flow of the fluid in and out of the cavity continuously flushes out obscuring blood or debris.
Hysteroscopy can be coupled with specialized surgical instruments small enough to fit through a small accessory port in the scope, thus allowing the surgeon to biopsy, destroy or remove lesions such as polyps or uterine fibroids.
Most of these procedures are performed in the office – no need for general anesthesia or a hospital stay. Dr. Garcini wants to get you well fast so you can get back to your normal activities with minimal disruption to your life.
Procedures that require small incisions
Laparoscopic procedures are those requiring entry into the pelvic or abdominal cavity. They are performed through one or more keyhole incisions that are created on the abdominal wall and typically do not leave scars. These procedures have traditionally been performed utilizing miniaturized instruments that allow the surgeon access to the pelvic cavity.
Most recently, the precision and flexibility of the surgeon has been augmented by use of the da Vinci® robot. Many gynecologic surgeries previously performed through large abdominal incisions are now routinely done laparoscopically. These include laparoscopic hysterectomy (removal of the uterus), laparoscopic oophorectomy (removal of one or both ovaries), laparoscopic myomectomy (removal of uterine fibroids) and excision of endometriosis lesions and pelvic adhesions.
These procedures are extremely safe, performed under general anesthesia in the hospital outpatient setting or at most requiring an overnight stay. Again, these procedures minimize postoperative pain and lead to less blood loss, fewer complications and faster recovery times.
