Laparoscopic hysterectomy & da Vinci® robotic surgery

Dr. Garcini is skilled and experienced with total laparoscopic hysterectomy (TLH). With TLH or the single-incision laparoscopic surgery (SILS), you can expect a much faster, easier recovery and less postoperative pain than with other approaches.

Robotic da Vinci hysterectomy is the latest method that brings 21st-century computer precision to the procedure. The da Vinci Robotic Surgical System® is widely touted as today’s most effective, least invasive surgical treatment option – an effective alternative to traditional open surgery. For you, that means improved precision, significantly less pain and a faster return to normal daily activities.

Robotic-assisted laparoscopic surgery

This technique involves the direct visualization of abdominal and pelvic organs via the introduction of a small fiberoptic lens 5-10 mm in size (with a built-in light source) into the abdominal cavity. Entry to the abdominal cavity is achieved by making a small incision at the level of the umbilicus (navel), inserting a multipurpose access port and passing the fiberoptic lens through the port into the abdomen. Nitrogen gas is used to fill the abdominal cavity, thus expanding the abdominal wall in order to obtain good visualization of the abdominal and pelvic organs.

Additional access ports may be placed in strategic places along the abdominal wall in order to facilitate the passage of multiple specialized, miniaturized surgical instruments. The net result is to permit the surgeon to perform intra-abdominal/pelvic surgery without the need of a large surgical incision. Intraoperative bleeding is less than in traditional “open” procedures. Postoperative pain is substantially decreased, and therefore postoperative recovery time is minimized. Many patients are discharged from the hospital the same day of the procedure.

Procedures traditionally performed through large abdominal incisions are now routinely handled through laparoscopic interventions. Examples of these procedures are diagnostic procedures, destruction and/or excision of endometrial lesions, laparoscopic hysterectomy (removal of the uterus), laparoscopic myomectomy (removal of uterine fibroids), surgeries for urinary incontinence and uterine prolapse, removal or drainage of ovarian cysts, removal of ovaries and/or fallopian tubes, bilateral tubal ligation and evacuation of ectopic (tubal) pregnancies.

A specialized form of laparoscopy called single-incision laparoscopic surgery, or SILS, is designed for those individuals who desire the esthetic benefits of having their entire laparoscopic surgery done through a single small umbilical (navel) incision.

The use of robotic-assisted laparoscopic surgery plays an important role in complex laparoscopic gynecology cases. The da Vinci robotic platform manufactured by Intuitive Surgery® enables the laparoscopic surgeon to see the operative field in a 3-D enhanced fashion and allows magnification of the operative field up to 10-fold. In addition, the surgeon-controlled robotic arms allow the laparoscopic surgeon to perform complex surgical maneuvers with great precision and ease not previously available in traditional laparoscopic surgery.

As a result, complex cases that would have required a large abdominal incision can now be performed with greater ease in a laparoscopic fashion. Robotic-assisted laparoscopic surgery has the same benefits as traditional laparoscopic surgery in terms of significant reduction in bleeding, postoperative pain and postoperative recovery time.

Abnormal PAP

The Pap test is an important part of our well-woman exams because it's the best way to detect changes in the cervix that could indicate cervical cancer. For the majority of women, the Pap test comes back normal. Even when the test does indicate a problem, often the cause is inflammation of the cervix or a vaginal infection, but you still need timely follow-up care.

Permanent Contraception

When you are done having children, Essure is the most effective permanent birth control available (even more effective than tying your tubes). Besides being surgery-free and hormone-free, Essure is performed in as little as 10 minutes with no slowing down to recover so you can get back to your family right away. Discover why women don't just depend on Essure, they love Essure.

Minimally Invasive Gynecologic Surgery

If there is ever a time when you need gynecologic surgery, it's good to know that there are now minimally invasive surgical solutions for most gynecologic problems. Dr. Garcini now performs 90% of his surgeries using these techniques. The reason is simple. These surgical solutions require either tiny incisions or no incisions at all. There is less pain for the patient, much shorter recovery times, less blood loss and far fewer complications - exactly what Dr. Garcini wants for all of his patients..

Heavy, Painful Periods

If heavy, painful menstrual cycles cause you to miss work or social activities every month, please talk with Dr. Garcini. He is a board-certified gynecology doctor with 17 years of experience evaluating and treating this common and very treatable health problem.

Pelvic Pain & Bladder Problems

Chronic pelvic pain not only hurts, it can be a major source of worry and keep you from leading a normal, fulfilling life. Even if you've been told that nothing can be done about your pelvic pain, we encourage you to schedule a visit. Our board-certified gynecology doctor, Francisco J. Garcini, MD, has 17 years of experience diagnosing and treating pelvic pain. He offers comprehensive testing, including in-office gynecologic ultrasound, to diagnose the cause of your pelvic pain. He will investigate your medical history and listen to you about your symptoms, previous treatments and anything else about your life that may be related.

Menopause

Menopause is a retrospective diagnosis. It is defined as the time when 12 consecutive months have passed since your last period, and it is a biological process that is characterized by a series of events that can start as early as your late 30s and last into your 50s and 60s. Physiologically, menopause is caused by the gradual decline in the production of estrogen by the aging ovaries. This state of estrogen deprivation is associated with hormonal, physical and psychological changes in your life; however, it is not the end of your youth or sexuality.
